To our Unicare patients, please ignore the copay your EOB is displaying, or any balance due reflected on it. Refer to your Agape bill for any copays due. Unfortunately, Unicare has us listed incorrectly, and not as a specialist. This is in the process of being corrected.

- Explanation as to the requirements: We believe that this is a necessary update to help protect your practice and patient information. Once Apple releases a new operating system, it no longer supports its older iOS versions, and it is not possible to ensure that unsupported devices and operating systems remain compliant with industry security standards.
Any telehealth appointment that needs to be cancelled due to incomplete or late check-in resulting in the appointment not being able to commence could result in a $75 late cancellation fee.
